We’re currently hiring a Full-Time OT to join our South Burlington Visits Office , located in Williston, VT , serving adult and geriatric clients across Chittenden County . This is your chance to build real relationships and deliver life-changing care where it matters most—at home.

What You’ll Do:

• Provide in-home care to clients in Chittenden County
• Conduct thorough functional assessments: strength, coordination, ADLs/IADLs, cognition, communication, and more.
• Design and deliver individualized care plans to help clients regain independence.
• Recommend adaptive equipment and modifications to improve safety and mobility.
• Educate clients and families on daily living strategies and safety techniques.
• Collaborate with the interdisciplinary care team and adjust care plans as needed.
• Document care using a digital tablet and easy-to-use EMR system.

What We’re Looking For:

• Licensed Occupational Therapist in Vermont.
• At least one year of recent OT experience (home health experience is a plus, not a must).
• Graduate of an accredited OT program.
• Confident working independently with strong time management and communication skills.
• Comfortable using digital tools for documentation.
